Community Living Services has a division dedicated to providing supports and services that keep older adults and adults with physical disabilities in their homes and communities. The CLS Senior Services division offers home making, personal care, community living supports and respite to people living in Wayne, Oakland and Macomb counties. The Senior Service division also supports the family members of older adults through its adult day service called Parkside Pioneers. Parkside offers daytime social, rehabilitative, and community activities for older adults who rely on their family for care and provides a safe place that family can trust while they get the much needed break from care giving that they need.
The Senior Services division also offers benefits and resource coordination through its Mi Café program (helping people secure food stamps and Medicaid) and its Medicare Assistance program helps people secure Medicare part D insurance coverage for prescription co-pays.
CLS is also a partner in the emerging Aging and Disability Resource Center Project for southern and western Wayne County. The ADRC will support people across disability and aging populations to connect to resources, supports and services they need through it’s “no wrong door” approach.
CLS and The Center for Self-Determination are partnering with the Michigan Office of Services to the Aging and some of the local Area Agencies on Aging to train its network of contractors and providers on Person Centered Planning and Self-Determination. CLS is a recognized leader in these initiatives and is sharing its knowledge and expertise in the field of aging and long-term care.

AVAILABLE SERVICES

Personal Care Services: Assistance with bathing, grooming, dressing, bathroom needs, transferring,
eating, and walking as needed. |
In Home Respite Care: Assistance with activities of daily living to individuals in the absence of a primary care giver. |
Homemaker Services: Routine tasks such as laundry,
ironing
meal preparation, shopping, and light
housekeeping tasks. |
Home Delivered Meals: For adults that are homebound, providing at least one nutritional meal per day. |
Home Health Aide Services: Health oriented services prescribed by a physician such as: exercise regimens, medication and or special diets, changing non-sterile dressing, taking blood pressure, and other healthcare activities. |
Home Maintenance: Assistance with maintaining and repairing household items on a non-continuous basis. |
Checking on the homebound adult: Regular contact through phone and/or in-home visits to assure well being and provide companionship and social interaction. |
MI Choice Waiver: Medicaid covered services like those provided within a nursing home but are provided within the individual’s home (enrollment in MI Choice Waiver Program required). |

SELF-DIRECTED BUDGETS

|
A self-directed budget is controlled by you, along with trusted family and friends and is completely optional, but highly recommended. This allows you to have control of who, what, when, where and how supports are provided. It shows you how money is being spent and evaluates the benefit of the services being offered to you. CLS staff will work with you to identify support needs and develop your budget which includes the costs of all services received through CLS.
